Evidence-based research shows that the prehospital preventable death rate in the battlefield is 25%. In other words, the life of one in every four soldiers seriously injured on the battlefield could have been saved with proper response. This alarming statistic underscores the urgent need for advanced training and tools to improve survival rates and operational resilience.

Our Solution

Extreme Simulations is dedicated to drastically reducing this number by providing 3D printed manikins and specialized training tools. Our mission is to increase operational resilience and truly save lives.
Our co-founder is living proof. In October 2023, Lieutenant Colonel (Res.) Jonathan (Yonti) Bahat was called up to serve as deputy brigade commander. Our teams trained Yonti’s brigade extensively before they entered Gaza. On January 8th, Yonti suffered five bullet wounds from terrorists during battle. He managed to stay on his feet and reach his team, who provided basic medical care. He then received advanced medical care from the brigade’s paramedic and was evacuated to a hospital, an act which saved his life. Only 28 minutes passed from the moment of injury to his hospitalization.
Extreme Simulations is the official and exclusive medical simulation trainer of the Israeli Defense Forces (IDF) and has recently won a 4-year contract to train all the branches of the Israeli army.
Since the beginning of Operation Iron Swords in Gaza and leading up to Operation Rising Lion in Iran, our teams have trained over 200,000 soldiers within an unparalleled breadth of categories. We vigorously train frontline responders in all areas of emotional, cognitive, and active response, so that in the heat of the moment, their skills take over and not their feelings. Systemically training nearly all combat soldiers has helped drastically reduce preventable death rates in the battlefield to 6.7% (compared to 25% globally) and has helped reduce symptoms of PTSD and other trauma-induced mental disorders.

Our solutions cover the entire process from concept through prototype to market-ready products.

Extreme Simulations offers a line of products designed to replicate the emotional landscape of trauma. Our solutions rely on years of experience serving over 500,000 first responders across the world’s top defense and healthcare organizations, including the IDF, USAR, MDA, Dutch MOD, US Marine Corps, Police SWAT (Yamam), Israel’s Secret Service, and others. In September 2025, we will be launching our new market-ready manikin: The Preventable Death Rate 25 (PDR25). This highly versatile, pre-hospital trauma training manikin is designed to simulate a wide range of real-world emergency scenarios. Ideal for military, disaster relief, paramedics, and first responders, the PDR25 features a modular injury system that allows for customizable trauma simulations, including amputations, fractures, major facial injuries, and impalements. With realistic airway management, advanced ventilation options, and hemorrhage simulation, the manikin supports critical skills training in airway management, CPR, venipuncture, and trauma care. Built for durability, it offers a lifelike training experience to prepare responders for high-pressure situations, helping to improve organizational resilience and enhance first response capabilities.
